KB34828: "Index out of range[]" error message appears when running a Report Services document in MicroStrategy Developer 9.x

"Index out of range[]" error message appears when running a Report Services document in MicroStrategy Developer 9.x

SYMPTOM:
When a user attempts to run a Report Services document in Strategy Developer 9.x, the error below is thrown:

Sample Code/Error
Error(s) occurred while loading schema: DSSSQLEngine: Schema loading error: Population Exception: The object shown in the following hierarchy no longer exists in schema: -Link-Base Attribute FormCOM interface error.
Message from GetErrorInfo : Index out of range[].

CAUSE:
In this occurence, an Index out of range error is a symptom of a metadata inconsistency in an Attribute's forms.  It is very likely that the internal representation of the attribute contains X number of forms but its supporting Table object says that it contains Y forms (X not equal to Y)..

ACTION:
In order to track down the cause of the inconsistency, customers encountering this issue should make note of Schema-related changes, Object Migrations, or error messages prior to experiencing the problem.  Change Journaling, if it has been enabled, can help in preserving this information.
