SYMPTOM
A report is executed against SAP BW and the following error occurs:

Error: SQL Generation Complete
QueryEngine encountered error: ExecQuery failed. MDX Execution failed. Error type: JCO layer error. JCOStatement.executeQuery failed. java.lang.Exception: Error during MDX Query Execution
Exception during Bapi Execution: com.sap.mw.jco.JCO$Exception: (104) RFC_ERROR_SYSTEM_FAILURE: Field symbol has not yet been assigned.
The cube which this report uses contains a hierarchy node variable whose definition in SAP allows only one answer. This can be seen in the variable's properties in SAP Business Explorer Query Designer:

However, in Strategy it is possible to answer the prompt with multiple attribute elements.

The MDX query includes both attribute elements, where SAP expects only one. Consequently, the query fails in the database.

CAUSE
This behavior is the result of a known issue in Strategy 10.x.
Strategy prompts can specify a minimum and maximum number of answers. If there are too few or too many answers, the user will be asked to correct the answer before execution can proceed.
Hierarchy node variables in SAP are imported into Strategy as hierarchy prompts. This prompt type is considered an "expression prompt," where each answer is a conditional expression. ("Qualify on an attribute" prompts are another form of expression prompt.)
The minimum and maximum setting applies to expression prompts, but it applies to expressions added to the prompt answer. The prompt answer illustrated above contains two attribute elements, but only one expression: Item in list (Lord of the Flies, Brave New World). Even if the maximum number of answers is set to 1, Strategy will validate this prompt successfully and pass the query to SAP.
ACTION
An enhancement request has been logged to validate this type of prompt based on attribute elements rather than whole expressions. Contact Strategy Technical Support for an update on the status of this issue.
In the meantime, the descriptive text in the prompt may be changed to instruct users to answer with only one element.
