SYMPTOM:
User experiences the flash mode of ESRI map not loading. Error "The Adobe Flash plugin has crashed" shows when executing ESRI map in Flash mode:

If user turns on Firebug, as the screenshot above, the error received is "GET crossdomain.xml 404 Not Found".
CAUSE:
This issue is caused by an Adobe "Cross-Domain Policy". For security reasons, a Macromedia Flash movie playing in a web browser is not allowed to access data that resides outside the exact web domain from which the SWF originated.
For more information about this Adobe policy, refer to http://kb2.adobe.com/cps/142/tn_14213.html
ACTION:
According to Adobe policy file (http://kb2.adobe.com/cps/142/tn_14213.html), user needs to create a crossdomain.xml file and host the file in the root location of web server.
A sample crossdomain.xml file:
Java
<?xml version="1.0"?>
<!DOCTYPE cross-domain-policy SYSTEM "http://www.macromedia.com/xml/dtds/cross-domain-policy.dtd">
<cross-domain-policy>
<allow-access-from domain="*"/>
<site-control permitted-cross-domain-policies="all"/>
<allow-http-request-headers-from domain="*" headers="*"/>
</cross-domain-policy>
