KB43119: How to display the page-by element for PDF subscriptions in MicroStrategy 9.3.1

SYMPTOM:
When creating file subscriptions with or without bursting, the page-by element is not displayed in the delivered PDF report.  However, with all other output formats, the page-by element is shown.
CAUSE:
This is expected behavior. By default, the page-by element is not shown in delivered PDF files.
ACTION:
To display the page-by element, follow the steps outlined below:

1. Execute the report and go to Home > Export > PDF.
2. Click on 'Edit custom settings'.
3. Add the "&" macro to the Header section and click Apply.
4. Click 'Export', go back to the report page and save the report.

Now, subscriptions will include the page-by element when delivered as PDF.
